Home Health brings professional medical care directly to the patient in the comfort and familiarity of their own home. It is typically prescribed by a physician following a hospital stay, surgery, or a change in health condition that requires skilled clinical oversight.
Our Home Health partners provide registered nursing, physical therapy, occupational therapy, speech therapy, medical social work, and home health aide services. The goal is to help patients regain strength, manage chronic conditions, and remain safely in their home environment.
